Expanding the National Approach to CDEA Funded Projects

Projects funded by the 3-year grant (Expanding the National Approach to Chronic Disease Education and Awareness, CDC-RFA-DP21-2112) include:

National Campaign for Lymphedema Awareness

An estimated 1.2% of patients with a primary cancer diagnosis develop lymphedema.1 This project will involve two key phases. The first phase will synthesize and compile disease indicators and information about best evidence-based practices to prevent, diagnose, and treat lymphedema that will be disseminated to public health professionals. In the second phase, a comprehensive awareness campaign will be developed and implemented for the public and health care providers. The campaign will consist of promoting existing and newly developed educational materials related to lymphedema through social networks, medical association partners, and public service announcements. The public awareness campaign will focus on individuals with cancer, who are at the highest risk of developing lymphedema, and who have not yet had surgical intervention.

Recipient: Lymphatic Education and Research Network

Obstructive Sleep Apnea Awareness Project

An estimated 10% of the US. adult population has a moderate to severe case of obstructive sleep apnea.2 The majority of adults are undiagnosed.3 This project will promote awareness to public health professionals through data dissemination activities, to the public through educational activities that include tailoring messaging specifically for designated health disparity populations, and to health care professionals through educational activities that focus on testing procedures, diagnosis, referral options, treatment, and long-term management options. A key component of this project will be to form a coalition of partners and collaborators that will work together to make a widespread and measurable effect on obstructive sleep apnea public and professional awareness.

Expanding the National Approach to Hearing Loss Education and Awareness

Hearing loss affects 23% of Americans aged 12 years and older.4 This project will generate composite estimates of the prevalence of hearing loss at the US county level using existing high quality data sources. These estimates will be disseminated in an appealing and easy-to-use website. This website will serve as a focal point for a health information campaign to disseminate the surveillance estimates and accompanying best-practice information to the public and hearing loss professionals. Information will also be shared with hearing loss professionals and the public using a collaborative partner engagement and social media approach.

Improving Patient Outcomes Through Increased Awareness of Psoriasis as a Chronic Immune-mediated Systemic Inflammatory Disease

Psoriasis is a chronic, immune-mediated systemic inflammatory disease affecting more than 3% of adults aged 20 years and older in the United States.5 This project will raise awareness among public health professionals, health care providers, and the public about psoriasis through several activities. This project will promote awareness to public health professionals through data dissemination activities, to the public through educational activities that focus on psoriasis as a systemic disease with significant comorbid conditions, and to health care professionals through educational activities that focus on addressing psoriasis diagnosis and undertreatment and present evidence-based interventions to screen and manage related comorbidities. The project will focus on the following populations to address health disparities: persons of color (e.g., Black, Latinx, Asian-American and Pacific Islander), transgendered individuals and members of the LGBT community, individuals with lower socioeconomic status, individuals living in rural communities, Medicare beneficiaries, and individuals aged 65 years and older. A dedicated webpage will serve as a resource for project information and offerings.

Addressing Chronic Lung Disease Project

An estimated 6% of the US adult population has chronic obstructive pulmonary disease (COPD).6 COPD accounts for the majority of deaths from chronic lower respiratory diseases, the fourth leading cause of death in the United States in 2019.7 This project will use a coordinated, comprehensive public health approach to improve prevention, early diagnosis, treatment, and management of COPD, by engaging a network of strategic community partners to increase knowledge and adoption of evidence-based strategies that improve COPD morbidity and mortality across the United States. Target audiences include public health professionals and healthcare professionals with access to high-burden populations, people living with COPD, and their families.

AChieving Equity for Disease prevention in Cystic Fibrosis (ACED-CF)

Cystic fibrosis (CF) is an inherited, lifelong disorder that requires medical care throughout the lifespan.8 Advances in CF diagnosis and treatment have transformed its prognosis from rapid progression and childhood mortality to a chronic multisystem disease of adults with rapidly increasing life expectancy and health span.8 This project aims to overcome current delays in initiating treatment for CF by disseminating study findings and distributing state-specific CF Foundation Patient Registry data on care processes and patient outcomes to state newborn screening program personnel and CF Center Directors. Through partnering with nonprofit institutions and academic collaborators, this project will (a) develop a public awareness campaign focused on newborn screening and the need for timely evaluation after out-of-range test results and (b) develop and implement continuing medical education and quality improvement activities for primary care pediatricians.
