Methods for State Oral Health Surveys that began data collection in 2008

Idaho

The survey was conducted during the 2008-2009 school year. The sample design for this survey used a probability-proportional-to-size (PPS) selection of public and charter schools with a 3rd grade enrollment of 22 or more. The sample was stratified by health district (7 health districts in Idaho). Schools that refused to participate were replaced with the closest school in size within the sampling strata. The examinations were completed by health district hygienists using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. In the state, 45% of elementary school students were eligible for the free/reduced lunch program (FRL); 37% of the students in the participating schools were eligible. The response rate was 87%. The data were weighted to account for the survey design. The estimates are presented adjusted and unadjusted for nonresponse.

Illinois

The survey was conducted during the 2008-2009 school year. The sampling frame consisted of public elementary schools students enrolled in 3rd grade. A probability sample of 100 schools was selected and 87 agreed to participate. Only those children that returned a positive consent form were screened. The statewide response rate was 52%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The data were weighted for the sampling scheme and adjusted for nonresponse.

Iowa

Head Start: The survey was conducted during the 2008-2009 school year. The sampling frame consisted of all Head Start classrooms in Iowa. A stratified random sample of 48 classrooms was selected and 43 agreed to participate. The screenings were completed by dental hygienists using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 800 children invited to participate, 631 were screened; for a response rate of 79%. The estimates presented are not adjusted for nonresponse.

Third Grade: The survey was conducted during the 2008-2009 school year. The sampling frame consisted of public elementary schools with 3rd grade. A random sample of 30 schools was selected and all 30 agreed to participate. The screenings were completed by dental hygienists using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the students selected, 65% responded. The percentage of children in the participating schools eligible for FRL was 38% while 34% of students in the state were eligible for FRL. The estimates presented are not adjusted for nonresponse.

Nevada

The survey was conducted during the 2008-2009 school year. The sampling frame consisted of all public elementary schools with 20 or more students in 3rd grade. The sampling frame was stratified by region (Washoe County, Clark County, all other counties) then ordered by percent of children that participate in the free/reduced school lunch (FRL) program. A probability sample of 42 elementary schools was selected. The screenings were completed using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. All 3rd grade children who returned a positive consent form were eligible to participate. Of the 4310 eligible students, 1786 were screened; for a response rate of 41%. The percentage of children in the participating schools eligible for FRL was 41% while 40% of the elementary school children in the state were eligible for FRL. The estimates presented are adjusted for the sampling scheme and non-response.

New Hampshire

The survey was completed during the 2008-2009 school year. The sampling frame consisted of all public elementary schools with 15 or more students in 3rd grade. With stratification by region and implicit stratification by percent of students eligible for the free/reduced price school lunch (FRL) program , a systematic sampling scheme was used to select 81 schools of which 81 agreed to participate. Trained dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 4725 eligible students, 3015 were screened; for a response rate of 64%. In the state, 22% of public elementary school students participate in the FRL program while 25% of the students at the participating elementary schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

Virginia

The survey was completed during the 2008-2009 school year. The sampling frame consisted of all public elementary schools with 3rd grade enrollment. With implicit stratification by race/ethnicity and percent of students eligible for the free/reduced price school lunch program (FRL), a systematic probability proportional to size sampling scheme was used to select 200 schools. Eight calibrated dentists and dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 15191 eligible students, 7838 were screened; for a response rate of 52%. In the state, 34% of public elementary school students participate in the free/reduced lunch program (FRL); 29% of the students screened were eligible for FRL. The estimates presented are adjusted for the sampling scheme.

Wisconsin

The survey was completed during the 2008-2009 school year. The sampling frame consisted of all Head Start programs in WI including tribal and migrant/seasonal programs. The sampling frame was stratified by the Department of Health Services five regions then probability proportional to size sampling was used to randomly select programs within each region. Trained examiners completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 1443 eligible children, 1212 were screened; for a response rate of 84%. The estimates presented are adjusted for the sampling scheme and nonresponse.
