Heart Disease Death Rates for 2011 through 2013 for Hispanics Aged 35 Years and Older by County. The map shows that concentrations of counties with the highest heart disease death rates - meaning the top quintile - are located primarily in Hawaii, New Mexico, and Texas. Pockets of high-rate counties also were found in parts of Michigan, New York, and California.
