Process Evaluation of a Regional Public Health Model to Reduce Chronic Disease Through Policy and Systems Changes, Washington State, 2010–2014

The state was divided into 5 hub regions. The east region counties were Ferry, Stevens, Pend Orelille, Lincoln, Spokane (hub lead and Community Transformation Grant [CTG] target county), Adams, Whitman, Columbia, Garfield, and Asotin. The central region counties were Okanogan, Chelan, Douglas, Kittitas, Grant (hub lead and CTG target county), Yakima (CTG target county), Klickitat, Benton, Franklin, and Walla Walla. The Northwest region counties were Whatcom (hub lead and CTG target county), Skagit (CTG target county), San Juan, Island, Clallam, Jefferson, and Kitsap (CTG target county). The central west region counties were Grays Harbor (hub lead and CTG target county), Mason, Thurston (CTG target county), Pacific, and Lewis (CTG target county). The southwest region counties were Wahkiakum, Cowlitz (CTG target county), Clark (hub lead and CTG target county), and Skamania. Three counties were independently funded: Snohomish, King, and Pierce.
