簡介
疾病控制與預防中心的國家糖尿病預防與控制計劃(National Diabetes Prevention and Control Program)及其屬下的59個糖尿病預防與控制計劃(Diabetes Prevention and Control Program)於1999年採納了[2010年健康人](Healthy People 2010)的五個目標。這些目標的目的在於提高糖尿病病患中預防性保健服務率,並包括每年一次的足部檢查、血紅素A1c檢測和每年一次的散瞳眼睛檢查。本文考查了達到這三個目標過程中的進展。

方法
我們使用行爲危險因素監控系統(BRFSS)中糖尿病單元的問題,評估了自2000年到2003年44個轄區每年一次的足部檢查、血紅素A1c檢測和每年一次的散瞳眼睛檢查等年齡調整後的檢查率的變化。糖尿病單元的問題還用於將2003年47個轄區的百分比與[2010年健康人]的目標百分比進行比較。

結果
自2000年到2003年,44個管轄地區每年一次的足部檢查累計年齡調整後的檢查率從63.7%提高到了69.3%(P < .001);自我報告的血紅素A1c檢測率從68.3%增加到了69.5%(P = 0.35);每年一次的散瞳眼睛檢查則從67.7%降到了65.2%(P = 0.05)。2003年,47個管轄地區中的20個達到了[2010年健康人]爲足部或眼睛檢查設立的目標,而且所有的47個管轄地區都達到了血紅素A1c檢測的目標。我們還發現所有三個國家糖尿病目標,其2000年起始檢查率和2000年至2003年的變化程度之間都存在著逆相關關係。

結論
國家糖尿病預防與控制計劃應該考慮採納更多的[2010年健康人]中的目標。在1)選擇目標;2)設定百分比目標;以及3)評估當前或未來的目標時應該考慮起始檢查率的水平。計劃相關的資訊應與傳統資料源如BRFSS結合使用,這樣我們才能瞭解環境因素的作用以及根據國家糖尿病目標評估轄區的進展情況。
