簡介
我們研究了患有糖尿病的成人之糖化血紅蛋白(HbA1c)值,以識別在患者、醫生和診所各水準上糖尿病醫療的品質變異,哪一水準造成最大的變異以及每一水準的哪些變數與糖尿病醫療的品質相關。

方法
研究受試者是18處診所的120位初級保健醫生及其2589名合乎要求的成年患者。因變數是記錄在臨床資料庫內的HbA1c值。使用了多變數分層模型來在患者、醫生或診所各水準對HbA1c值進行分類,以識別各水準上HbA1c的重要預測變數。

結果
HbA1c值超過95%的變異是由患者水準造成的。醫生和診所水準的變異顯著較少。包括患者和醫生共變數在內並為顯著減少這種結果模式。強化藥物治療( t = −7.40, P < 0.01)和患者年齡(t = 2.10, P < 0.05)與HbA1c有利變化相關。醫生年齡、醫生專業、每位醫生的糖尿病患者數目、患者的共病狀況以及診所指派都不能預測HbA1c值的變化。共變數的總體模型造成了隨時間的HbA1c值變化的11.8%。

結論
這些資料提示,儘管醫生在某些患者因素中也發揮了重要作用(如強化藥物治療),但是HbA1c值最大的變異是由患者因素造成的。這些結果可能可以帶來更有效的醫療改善策略和責任措施。
