背景
糖尿病的發病率已達到流行疾病的程度了。糖尿病自主管理教育(DSME)已經顯示可以改善預防性保健行爲的效果和臨床結局。在本研究中,我們討論了在阿肯色州醫療落後的農村地區實施DSME計劃時面臨的障礙。

基本情況
阿肯色是個農業州,其東南部的大部分地區都存在著醫療專業人員缺乏的問題。阿肯色州糖尿病預防與控制計劃(Arkansas Diabetes Prevention and Control Program)及其合作夥伴在糖尿病流行程度嚴重的落後地區成立了12個DSME計劃。

方法
DSME計劃的提供方式是由一名註冊護士和一名營養專家向每位計劃參與者提供10到13個小時的教育培訓。新近成立的DSME的參與者於起始、6個月和一年結束的時候收集有關預防性保健行爲的資訊,如每日血糖監控、足部檢查、血壓舒張壓和收縮壓水平以及糖化血色素水平等。

結果
在成立的12個DSME計劃中,有11個獲得了美國糖尿病協會的認可。DSME計劃的參予人數從2003年2月的308人增加到2004年3月的734人,在一年的時間了提高了138%。預防性保健行爲也得到了改善:參與者的每日血糖監控率從56%上升到了67%,每日足部檢查率從63%增加到了84%。每位完成整個計劃的參與者糖化血色素平均降低了0.5個單位。但是,本計劃的實施過程遇到了許多預料中和一些不在預料中的障礙,而且由於缺乏評估方案,這些障礙無法克服。

說明
儘管結果顯示本計劃對DSME參與者預防性保健行爲方面有潛在的益處,但是由於樣品數量不夠局限了結果的說明。樣品數量不足可與評估計劃結果的障礙聯繫起來。計劃的評估應整合入計劃的籌劃階段,以確保可以充分衡量計劃的有效性。
