• Population Estimates Survey (PES)
  Data provided through this survey is vital to funding to support purchase of vaccines. Additionally, data obtained from the PES is used to make projections and analysis regarding your vaccine needs and monitoring and is used to populate your Vaccine Forecasting Application.

• VFC Management Survey
  This web-based questionnaire is required by March 1 annually for the previous calendar year’s activities. Information collected includes: number of enrolled providers and how the VFC program operates within the awardee’s geographic area, VFC/IQIP activities, and a report on VFC accountability activities.
