2013 Funding (Year 4)

    Awardee will:
    • Integrate quality improvement into department protocols
    • Update the self-assessment for accreditation readiness
    • Conduct a mock review of accreditation documentation
    • Support workforce development training
    • Convene internal and external partners on public health improvement and accreditation

2012 Funding (Year 3)

  • Amount of funding: $250,000
  • Description of Project
    Awardee will:
    • Evaluate accreditation documents
    • Organize internal policy reviews and updates for accreditation
    • Update and apply balanced scorecard targets/measures
    • Convene quality improvement projects across its divisions
    • Engage the community and partners in health assessment use

2011 Funding (Year 2)

  • Amount of funding: $249,998
  • Description of Project
    Awardee will:
    • Evaluate infrastructure and programs using the newly established iPEX (Improving Programs and Processes for Efficiency and Excellence) performance management system
    • Engage in ongoing performance improvement projects using the iPEX performance management system
    • Develop a county health assessment and county health improvement plan
    • Hire an accreditation coordinator dedicated to focusing on the department's accreditation activities
    • Provide evidence-based performance management training to a leadership team, representing multiple divisions across the health department, to help accelerate broad implementation of best practices for accreditation readiness

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
