2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Focus on strengthening accreditation readiness in three domains and working toward a Public Health Accreditation Board-compliant state and community health assessment
    • Expand and improve its performance management system and performance dashboard application
    • Improve service delivery in its local health clinics
    • Establish statewide quality improvement (QI) plan
    • Increase QI capacity through a community of practice and training

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Fully develop and deploy a robust performance management data system with quality improvement (QI) incorporated
    • Train staff at all organizational levels in performance management, QI, and broader public health competencies
    • Complete QI projects that focus on clinical operations broadly in family planning program administration and clinical operations and in sexually transmitted disease clinic delivery
    • Implement an organizational self-assessment based on the Public Health Accreditation Board (PHAB) standards and measures
    • Complete a Healthy People 2010/2020 report, which will serve as its state health assessment, a PHAB accreditation prerequisite
    • Support and coordinate community-level assessments and improvement plans

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Augment the quality improvement capacity and operations of the Department of Health and Environmental Control (DHEC), its eight public health regions, and its 46 county health departments
    • Support the agency's purchase of Q Flow, a clinic management application that provides real-time metrics for measuring wait and transaction times, total service time, and abandonment/diversion rates, as well as an enterprise view of productivity, service and performance levels, and customer flow patterns
    • Conduct an organizational self-assessment of health services quality improvement that will lead to the development of specific strategies and activities to help DHEC meet several performance measures within the Public Health Accreditation Board's Domain 9
    • Increase in the number of promising and best practices implemented within the agency, which will be shared with the CDC performance improvement network

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
