2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Implement year two of its strategic plan
    • Begin implementing the state health improvement plan in collaboration with a newly established public health improvement advisory council
    • Establish teams and processes to gather, review, and organize documentation and to further prepare for accreditation
    • Create a customer satisfaction feedback system using best practices from around the country and building on existing efforts within the department
    • Develop a workforce development plan in conjunction with human resources and Connecticut schools of public health
    • Conduct quality improvement projects aimed at increasing efficiencies of drinking water surveying and the rendering of decisions for certificate of need applications using lean processes and plan, do, study, act methods for improvement
    • Launch a performance management system information technology application
    • Offer online and hands-on quality improvement and leadership development training to staff that can be tracked through TRAIN

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Complete the three accreditation prerequisites
    • Complete an accreditation self‐assessment and develop a roadmap to accreditation
    • Implement three quality improvement (QI) processes
    • Continue QI training and accreditation workshops for public health professionals
    • Build performance management and QI into the agency through development of a quality improvement plan and policy and development of a Quality Improvement Council

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Develop an agency strategic plan
    • Conduct a statewide assessment of health status and health services
    • Collaborate with multi-sector stakeholders to develop a comprehensive state health improvement plan
    • Increase the number of initiatives to implement evidence-based policies, regulations, and laws, and best and promising practices
    • Hire a full-time "Performance Improvement Manager" and part-time epidemiologist to support the activities of the Performance Management Office and move the agency toward meeting national accreditation standards

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
