2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Finalize a statewide community health assessment
    • Form a lead committee to begin work on and create an action plan for completing a state health improvement plan
    • Conduct a grants management system quality improvement initiative that will identify and document existing processes, provide grants management education, produce a grants management handbook, and lay the foundation for standardization of all grants management processes
    • Increase the number of individuals accessing public health information on its website through a documented system enhancement, implementation, and tracking of complementary social media sites such as Facebook and Twitter and the introduction of metrics for learning why people return to the website or social media sites
    • Increase performance accountability and performance tracking at the Division of Public and Behavioral Health

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Establish a state-level performance management office
    • Conduct organizational self-assessment against Public Health Accreditation Board standards and measures to identify and begin to address gaps at the state level
    • Finalize statewide core indicators for community health status assessments and use the list to complete a draft of a state community health assessment
    • Increase the percentage of clients who are satisfied or very satisfied with wait times for local-level clinic services
    • Increase the percentage of clients served at local clinics who have health insurance coverage and increase the revenue generated by those billable services

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Implement relevant and essential activities to accelerate its accreditation readiness
    • Develop a state/tribal/community health assessment
    • Prioritize activities, practices, and programs for maximum impact
    • Eliminate activities, practices, and programs with limited or no public health impact
    • Engage in cross-jurisdictional partnerships with one or more local health departments to increase the number of initiatives that implement evidence-based policies, regulations, and laws
    • Increase the number of promising and best practices implemented within Nevada

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
