Speaker Request

Note:  We are currently experiencing some technical difficulties with the Speakers Bureau site.  When submitting your request for the CDC Director or other speakers, please do not include any special characters or symbols such as commas, asterisks, plus signs, periods, dashes, etc., to ensure that your request is successfully entered into the system.  You may also submit your request to providing the event overview, date, contact information, and other pertinent information as outlined in the form.  We apologize for the inconvenience.

While some details may not be immediately available, we appreciate you making every effort to submit this form at least six weeks before your event. Requests of extremely short notice (within 10 days of the event) may not be processed since it will not allow for adequate staffing coordination.
