The Guide to Community Preventive Services (Community Guide) is a free, evidence-based resource to help choose programs and policies to improve health and prevent disease in your community.

"An Organizing Framework for Translation in Public Health: The Knowledge to Action Framework" The Knowledge to Action Framework was designed to capture all the processes involved in translation, beginning with discovery through the decision to translate through institutionalization. This journal article discusses the development of the framework.  

Dissemination Planning Checklist [PDF-254KB] This checklist is developed from the Knowledge to Action Framework and is intended to serve as an example of a tool that can assist PRCs in dissemination planning.
