Methods for State Oral Health Surveys that began data collection in 2014

Arizona

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all non-reservation public and charter schools with 20 or more children in kindergarten and/or 3rd grade. With explicit stratification by county and implicit stratification by percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic probability proportional to size sampling scheme was used to select 104 schools. Data is available for 84 schools. Of the 14,933 kindergarten and 3rd grade children in the 84 participating schools, 6003 were screened for a response rate of 40%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. For the schools in the sampling frame, 58% of children are eligible for the FRL program; among participating schools 72% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse

Florida

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all Head Start centers. With implicit stratification by region and county, a systematic probability proportional to size sampling scheme was used to select 29 Head Start centers. Of the 1535 children invited to participate, 1433 were screened for a response rate of 93%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The data were weighted for the sampling scheme and adjusted for nonresponse.

Hawaii

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all public elementary schools with 20 or more students in 3rd grade. With explicit stratification by county and implicit stratification by percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic probability proportional to size sampling scheme was used to select 64 schools. Data is available for 67 schools, 58 of the originally selected schools, 6 replacement schools and 3 extra schools on Molokai. Of the 5541 3rd grade children in the 67 participating schools, 3184 participated for a response rate of 57%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. In the state, 53% of elementary schoolchildren are eligible for the FRL program; among participating children 53% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.

Iowa

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all Head Start centers. A probability proportional to size sampling scheme was used to select two Head Start centers from each of the 24 Title V child health service areas. Of the 1431 children invited to participate, 1090 were screened for a response rate of 76%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The data were weighted for the sampling scheme.

Minnesota

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all public schools with 10 or more children in 3rd grade. With implicit stratification by percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic probability proportional to size sampling scheme was used to select 50 schools. Data is available for 50 schools. A passive/opt-out consent process was used. Of the 3711 3rd grade children in the 50 participating schools, 3199 were screened for a response rate of 86%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. For the schools in the sampling frame, 43% of children are eligible for the FRL program; among participating schools 41% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.

North Dakota

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all public and Bureau of Indian Education elementary schools with 5 or more students in 3rd grade. With implicit stratification by urban/rural status of the county and percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic sampling scheme was used to select 47 schools. Data is available for all 47 sampling intervals. Of the 2,641 3rd grade children in the 47 participating schools, 1995 participated for a response rate of 76%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. In the state, 35% of elementary schoolchildren are eligible for the FRL program; among participating schools 35% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.

Virginia

The survey was conducted during the 2014-2015 school year. The sampling frame consisted of all public schools with 20 or more children in 3rd grade. With explicit stratification by region and implicit stratification by minority enrollment and percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic probability proportional to size sampling scheme was used to select 160 schools. Data is available for 149 schools. Of the 15,322 3rd grade children in the 149 participating schools, 5682 were screened for a response rate of 37%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. For the schools in the sampling frame, 45% of children were eligible for the FRL program; among participating children 38% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.
