Methods for State Oral Health Surveys that began data collection in 2003

Colorado

The survey was conducted during the 2003-2004 school year. The sampling frame consisted of all public elementary schools with a 3rd grade enrollment of 10 or more. With implicit stratification by percent of students eligible for the free/reduced school lunch program (FRL), 50 schools were randomly selected. If a school refused to participate, a replacement school within the same sampling strata was selected. If the sample school plus two replacement schools refused to participate, no data were collected in that sampling stratum. Of the 50 stratums, data is available for 44. All children in both kindergarten and third grade were screened; if they returned a positive consent form. Trained volunteer hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the students selected, 68% of the third grade and 74% of the kindergarten children responded. The percentage of children in the participating schools eligible for FRL was 34% while 33% of elementary students in the sampling frame were eligible for FRL. The data were self-weighting and the estimates are presented adjusted and unadjusted for nonresponse.

Illinois

The survey was completed during the 2003-2004 school year. The sampling frame consisted of all public elementary schools in Illinois with 3rd grade enrollment. A stratified systematic probability proportional to size sampling of schools, with stratification by region, urbanicity, county (in the West Chicago collar region), and socioeconomic status of the schools, was used to select 101 schools. Trained dentists and dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the students selected, 66% responded. The percentage of children in the participating schools eligible for FRL was 54%, slightly higher than the percentage of children eligible for FRL in the sampling frame (47%). The data were adjusted for the sampling scheme and nonresponse.

Kansas

The survey was conducted during the 2003-2004 school year. A self-weighting sample of public elementary schools with at least 10 children in third grade was obtained for each of the five regions in the state. Within each region, elementary schools were sorted by percent of children eligible for the free or reduced price meal program. A random sample of elementary schools then was selected for each region. Eighty schools were selected but only 47 agreed to participate. All 3rd grade children in the school were eligible to participate if they returned a positive consent form. The response rate in the participating schools was 31.5 percent. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The screenings were completed by eight dental hygienists and one dentist who participated in a training and calibration session. Estimates were not adjusted for nonresponse.

New Hampshire

The survey was conducted during the 2003-2004 school year. The sampling frame consisted of all public elementary schools with 3rd grade. Schools were selected with probability proportional to 3rd grade enrollment size. One 3rd grade class in each school was randomly selected and all students in that class were eligible to participate if they returned a parent consent form. Of the 46 schools selected, 42 participated. Of the 872 eligible students, 597 were screened; for a response rate of 68%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The data were weighted for the sampling scheme and adjusted for nonresponse.
