Office Visit: Patient Sally

  • Sally is a 17 year old previously healthy female who comes to see her primary care provider at a clinic.
  • History of Present Illness:
    • Two month history of severe migraines with some nausea and vomiting. OTC medication provides temporary relief.
    • Difficulty sleeping
    • Fatigue and daytime sleepiness for the past three weeks. Not able to focus on school work.
    • Burn on right arm sustained about one week ago. It was treated with over-the-counter burn cream. Sally would also like to have her burn evaluated and treated.
