Laborer crushed under overturned road milling machine.

On September 30, 2003, a 24-year-old construction laborer was killed when he was crushed under an overturning road milling machine. The incident occurred at the construction site of a new taxiway at a large international airport. The construction company was using a large, diesel-powered milling machine (profiler-reclaimer) to remove an old concrete taxiway before building the new taxiway. This tracked machine used a rotating milling drum to remove a 12' wide strip of pavement to a predetermined depth. The machine is guided forward by the operator as two groundsmen walk along each side of the machine to monitor and adjust the cutting depth. At the time of the incident, the machine operator had finished milling a section of pavement and was backing the machine into position to mill the next section. As the operator moved the machine into place, the tracks went up onto an elevated section of pavement, rocks, and debris. This caused the machine to tip sideways too quickly to allow the operator to compensate with the machine's controls, and the machine rolled over onto its side. Unknown to the operator, the victim, who was acting as a groundsman, was along side of the machine as it rolled over. He was crushed and killed instantly under the machine. The operator and second groundsman were not injured in the incident. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Machine operators must be trained to ensure that equipment is operated within the manufacturer's design specifications. 2. Employers must ensure that machinery is operated in accordance with manufacturer's directions. 3. Milling machine manufacturers should install active warning devices to alert the operators when the machine's slope limitations are being approached.
