Cumulative trauma disorders (CTD) in a poultry plant, Georgia.

A survey of the incidence of cumulative trauma disorders (CTDs) among poultry dressing workers was conducted by NIOSH in May 1988 among the workers in a large poultry processing factory in Georgia. The facility employed 409 hourly employees, mostly women, in the deboning department. Incidence rates for upper extremity CTDs were calculated using 1988 OSHA 200 logs maintained by the company. A questionnaire was completed by female employees to report on their health and work history and to describe the location, duration, onset, and aggravating factors of any upper extremity symptoms. They were given a physical examination focusing on symptoms and signs in the neck and upper extremities. The prevalence of upper extremity CTDs in the facility was determined from the questionnaire responses and the physical examination findings. The factory as a whole had a CTD incidence rate of 35 cases per 100 fulltime workers per year. By department, the deboning department had the highest rate 49 cases/100. By specific job, breast trimming had the highest rate, 125 cases/10. The hands and wrists were the most frequently affected, followed by the arms and shoulders. Thirty five workers were judged to have upper extremity CTDs based on the questionnaire and physical examination findings, yielding an overall point prevalence of 31%. The most affected joint areas were hand/wrist 27%, neck 10%, shoulder 6%, and elbow 1%. Tendinitis and tension neck syndrome were the most frequent diagnoses, 42 and 21% of the workers being diagnosed with these conditions, respectively. Employment in jobs involving high exposure to repetitive motion and forceful wrist movements was a significant risk factor for a CTD. The authors conclude that a high rate of upper extremity CTDs, particularly tendon related disorders, has been found in this facility.
