A ten-year follow-up study of cotton textile workers.

A study of the health effects of chronic exposure to cotton dust was conducted through a 10 year followup to a 1975 study of workers at a cotton textile mill in Yugoslavia. The cohort consisted of 37 nonsmoking female and 31 smoking male workers. The original cohort consisted of 72 females and 44 males. The subjects completed a respiratory symptom questionnaire that included questions on byssinosis. Spirometric testing was performed. Industrial hygiene sampling for total and respirable cotton dust was conducted. The prevalence of chronic cough, chest tightness, and byssinosis was significantly higher in females after the 10 year followup than in the original study, 31.4 versus 11.4%, 80.0 versus 51.4%, and 42.9 versus 22.9%, respectively. The prevalence of chronic cough, chronic phlegm, chronic bronchitis, and byssinosis in male workers after followup was significantly higher than originally, 77.4 versus 45.2%, 70.9 versus 38.7%, 64.5 versus 19.4%, and 51.6 versus 25.8%, respectively. Significant cross shift decreases in 1 second forced expiratory volume (FEV1) and forced vital capacity (FVC) occurred in female workers and male workers with byssinosis in the original and followup study. Mean annual decreases in FVC and FEV1 were significantly larger than expected in all subjects. Female workers who had byssinosis in the initial study had larger mean annual decreases in FVC and FEV1 than those without byssinosis. There were no differences in mean annual FVC and FEV1 decreases between male workers with and without byssinosis. Mean total and respirable cotton dust concentrations were 3.95 and 0.97mg/m3, respectively. The authors conclude that the increased progression of byssinosis and other chronic respiratory symptoms in the cohort is almost certainly due to the observed high cotton dust concentrations and the susceptibility of the workers to the effects of cotton dust.
