Electrician electrocuted in Tennessee, August 20, 1987.

The case of an electrician who was electrocuted while trouble shooting a 480 volt DC generator that supplied power to a glue machine was examined. The victim worked for a small company that performed industrial electrical wiring and maintenance. The company has no safety program of its own but hired only union electricians, who received training. The victim was assisting in the relocation of a 480 volt DC generator. After reconnecting the electrical wiring, the victim turned on the power to the generator and asked the facility engineer to push the start button. When the generator failed to start, the victim took his voltmeter and verified that current was reaching the starter. The victim moved next to the rear of the generator to verify that he had made the electrical connections correctly and to check the two fuses located in the rear of the generator. The victim was found behind the generator, slumped over with one arm on a transformer. Resuscitation efforts were unsuccessful. Based upon location of the equipment, the victim reached from the side of the generator, apparently contacting one phase of the 480 volt alternate current (AC) power supply. It is suggested that decisions about equipment location should include consideration of maintenance requirements. It was recommended that this company develop a safety program that stresses hazard recognition and safe work procedures.
