A self-evaluation questionnaire designed for use in the nitrogen fertilizer industry (SIC-287) is presented. The questionnaire was developed by occupational safety, industrial hygiene, and occupational medical personnel. The questionnaire identifies fertilizer industry operations and is designed to determine injury and illness potential for various work operations. Results of the questionnaire are used to establish safe work practices and control methods. Exposures to anhydrous ammonia (7664417), nitric-acid (7697372), urea (57136), and nitrogen (7727379) solutions are considered, along with neutralization processing and the manufacture of solid ammonium-nitrate (6484522) solution. The authors note that this questionnaire should be used in combination with the Self- Evaluation of Occupational Safety and Health Programs document.
