All NCHS staff and contractors and non-NCHS staff (agents) must annually sign the affidavit of nondisclosure to indicate a commitment to protect the confidentiality of NCHS data. The instructions for signing the affidavit of nondisclosure are different depending upon whether one works at NCHS facilities or not:

 

NCHS staff and contractors: Must electronically sign the affidavit of nondisclosure, using their PIV cards, to fulfill the annual requirement. The electronic copy will be saved to a SharePoint site managed by the NCHS Confidentiality Office. Additionally, a copy of the confidentiality training certificate of completion must be attached to the Affidavit of Nondisclosure. Non-NCHS Staff (Agents): Click here to open the affidavit of non-disclosure. Print and sign the affidavit of nondisclosure then submit the paper copy to your supervisor and/or data custodian.
