CDC-RFA-DP-23-0014: Racial and Ethnic Approaches to Community Health (REACH 2023)
CDC announces fiscal year 2023 funds to implement CDC-RFA-DP-23-0014: Racial and Ethnic Approaches to Community Health (REACH 2023). This 5-year program aims to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease. REACH programs work specifically among African American, Black, Hispanic, Latino, and Asian American, Native Hawaiian/Other Pacific Islander, American Indian, and Alaska Native populations.
Component A is required and focuses on:
- Food service and nutrition guidelines
- Fruit and vegetable vouchers and produce prescriptions
- Increasing physical activity through community design
- Continuity of care in breastfeeding support
- Obesity prevention standards in early care and education (ECE) programs
- Support for family healthy weight programs
- Tobacco prevention and control policies
Applicants must propose work in nutrition and physical activity plus another strategy for Component A. Component B is optional and focuses on flu, COVID-19, and other adult vaccination programs.
Funding is for a 5-year project period, pending the continued availability of funding.

Amendment: The letter of acknowledgement from the State Chronic Disease Director is not required.
