Program Success Stories

Funded communities in CDC's Healthy Communities Program are working to reduce the burden of chronic diseases and achieve health equity by implementing population-based strategies that create sustainable systems and environments. Communities apply evidence-based strategies to improve access to nutritious foods, increase physical activity opportunities, and reduce the initiation and consumption of tobacco use as well as exposure to secondhand smoke. Learn more—through stories and journal articles—about the research and success of local level changes brought about by communities.
