Closing the Gap in State-Level Diabetes Surveillance

CDC works with state health departments to track diabetes prevalence, control of diabetes risk factors such as prediabetes, and control of key indicators such as hemoglobin A1c, blood pressure, and cholesterol.  However, it is difficult to estimate diabetes prevalence (including undiagnosed cases), prediabetes, and related health problems at the state level.  This project examined the feasibility of using a variety of data sources in novel ways to address this challenge.
