Counties are shaded according to tertile of age-adjusted county-level diabetes incidence rates. Higher rate values are represented by darker shades of yellow.

Counties are shaded according to tertile of Area Deprivation Index (ADI) score. Higher index values represent higher levels of socioeconomic disadvantage (i.e., lower levels of socioeconomic status) and are represented by darker shades of blue.

ADI is a geographic area-based measure of the socioeconomic disadvantage experienced by a community. The index uses various markers of socioeconomic status including level of education, housing value, family income, and unemployment status. High disadvantage is associated with a higher risk for poor health, as well as poorer health care outcomes.
