Geographic Patterns: Heart Disease Death Rates for 2017 through 2019 for Whites Aged 35 Years and Older by County. The map shows that concentrations of counties with the highest heart disease death rates – meaning the top quintile – are located primarily in Alabama, Mississippi, Louisiana, Arkansas, Oklahoma, Kentucky, and Tennessee.  Pockets of high-rate counties also were found in Georgia, South Carolina, Missouri, California, Nevada, Arizona, Utah, Michigan, western New York, Texas, and New Mexico.
