Heart Disease Death Rates for 2011 through 2013 for Whites Aged 35 Years and Older by County. The map shows that concentrations of counties with the highest heart disease death rates - meaning the top quintile - are located primarily in Mississippi, Oklahoma, Louisiana, Alabama, Kentucky, Tennessee, and Arkansas. Pockets of high-rate counties also were found in Georgia, West Virginia, South Carolina, California, Nevada, Texas, and Missouri.
