Heart disease is the leading cause of death in Louisiana. Approximately 45% of the Louisiana population is within a thirty-minute drive time to the Population Health Cohort sites. This map demonstrates the geographic disparities in the burden of heart disease and identifies gaps in access to Population Health Cohort sites in Louisiana among some communities with heavy burdens of heart disease mortality.
