Program Locations

State of Minnesota

Background: Minnesota Department of Health was funded as a standard project in June 2004.

Lifestyle Intervention: Minnesota is proposing a lifestyle intervention that includes telephone counseling and directing women to community resources.

Screening: Risk factor screening includes blood pressure, cholesterol, and other lipids testing as well as assessing weight, tobacco use, poor diet, and physical inactivity.

Sites: Minnesota is planning to use a network of providers, primary care clinics, and community clinics to implement the program.

Key Partners: Proposed partners are the American Cancer Society, American Heart Association, and other divisions within the Department of Health.
