Program Locations
Background: Since 2000, Michigan Department of Public Health has
been funded as a standard project.
Lifestyle Intervention: Michigan uses a modified DASH diet that
focuses on eating more fruits, vegetables, whole grains and non–fat dairy
products along with physical activity recommendations. Participants
receive 1–5 lifestyle counseling contacts depending on screening results.
The intervention uses Lifestyle Contracts and incentives to support the
lifestyle behavior changes.
Screening: Risk factor screening includes blood pressure,
cholesterol and other lipids testing as well as assessing weight, medical
history, tobacco use, poor diet, and physical inactivity.
Sites: The Michigan Department of Community Health contracts with
nine
local health departments to implement the WISEWOMAN program. The program
is implemented in federally qualified health centers, local health care
providers, and local health department clinics.
