Program Locations

State of Iowa

Background: Since 2000, the Iowa Department of Public Health has been funded as an enhanced research project.

Lifestyle Interventions: Iowa uses Iowa Care for Yourself, based on the Dietary Approaches to Stop Hypertension (DASH) diet. Iowa Care for Yourself is a 12–session group format lead by Cooperative Extension health educators that includes a healthy snack, topic presentation, brief physical activity, skill–building activity, behavior change and goal–setting activities, and evaluation.

Screening: Risk factor screening includes blood pressure, glucose, cholesterol and other lipids testing as well as assessing weight, medical history, tobacco use, poor diet, and physical inactivity.

Sites: Fifteen sites including health departments, hospitals, visiting nurse associations, home health services, and women’s service agencies.

Key Partners: University of Iowa at Iowa City and Iowa State University Extension Services at Ames.
