California WISEWOMAN Program
Clinical Services: California WISEWOMAN works with health care providers to provide cardiovascular disease risk factor screenings to low-income, underinsured/uninsured women between the ages of 40 to 64 years. These screenings include blood pressure, glucose, and cholesterol as well as assessing height, weight, medical history, quality of life indicators, tobacco use, diet/nutrition, and physical activity. California WISWOMAN provides screening, medical support, and risk reduction counseling through contracts with three clinics: 1) Northeast Valley Health Corporation in Los Angeles County; 2) Vista Community Clinic in San Diego County; and 3) Family Health Centers of San Diego-Logan Heights clinic. These clinics were chosen based upon the greatest reach and screening potential. Clinics are reimbursed via fee-for-service, based on the number of risk reduction counseling, health coaching, lifestyle program, or follow-up sessions delivered each month.
Lifestyle Programs and Other Healthy Behavior Support Options: California WISEWOMAN delivers health coaching through bilingual, bicultural clinic staff. Clinic staff utilize core elements from the evidence-based Spanish-language intervention called Vida Saludable, Corazón Contento!, and the English-language version, “A New Leaf: Choices for Healthy Living,” along with the Manual del Programa Corazón de la Familia de California, during health coaching. The program also makes referrals to community resources to support physical activity, healthy eating, and smoking cessation, as appropriate.
