Download a VIS onto a Mobile Device
When you or your child gets a vaccine, law requires that you be offered a copy to take home of the appropriate VIS (Vaccine Information Statement).
(Why? VISs contain information that may be useful after you leave the doctor's office – for example, when the next dose of vaccine is due, or how to recognize or report a reaction.)
If you have a Mobile Device (for example, iPhone, Palm Pre, BlackBerry) that can display a pdf file, you may choose to download the VIS(s) to this device rather than take away paper copies. Click on the appropriate link(s) below to download.
If you prefer a paper copy, tell your provider.
