Private is made up of Private Clinics, HMOs and Group Practices.  Mixed is for a child that received vaccination from more than one type of provider (Private and Public).  Other is for all other type of providers combined (Hospitals, Military Facilities, and Unknowns.)
§ Children in the Q3/2001-Q2/2002 National Immunization Survey were born between August 1998 and November 2000.
