One or more doses of any diphtheria and tetanus toxoids and pertussis vaccines including
diphtheria and tetanus toxoids, and any acellular pertussis vaccine (DTP/DTaP/DT).

One or more doses of any poliovirus vaccine.

§ One or more doses of Haemophilus influenzae type b (Hib) vaccine.

ll One or more doses of hepatitis B vaccine.

¶ Two or more doses of hepatitis B vaccine. The second dose is recommended to be
administered between 1 and 4 months of age.
