2012 Funding (Year 3)

• Amount of funding: $993,662
• Description of Project
  Awardee will:
  • Support accreditation for state, local, and tribal health departments
  • Ensure quality improvement as a standard process to achieve measurable outcomes
  • Ensure the implementation of performance management processes to improve organizational practices and culture in measuring and reporting continuous improvement of outcomes for targeted populations in Healthy Wisconsin 2020 and the Wisconsin Department of Health Services Strategic Plan
  • Implement a review to cross-walk 140 statute requirements and national accreditation requirements for local and tribal health departments

2011 Funding (Year 2)

• Description of Project
  Awardee will:
  • Develop a culture of performance management and quality improvement throughout the state
  • Continue to center around accreditation by national standards, implementation of evidence-based practice, implementation of quality improvement methods, and use of technology for performance tracking

2010 Funding (Year 1)

Component I

• Amount of funding: $300,000
• Description of Project:
  Awardee will:
  
  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

• Amount of funding: $1,660,128
• Description of Project:
  Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
