• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Continue collaboration with the Utah Association of Local Health Departments, which represents the state’s 12 local health districts
  • Complete and begin implementation of a statewide health improvement plan
  • Conduct accreditation readiness self-assessment and develop an accreditation roadmap
  • Implement the following quality improvement projects: streamlining agency travel approval, improving quality culture at the Salt Lake Valley health department, and increasing reach and effectiveness of the smoking cessation program for pregnant women
  • Provide management and performance improvement boot camp training
  • Conduct a performance improvement project for online health assessments and lessons learned

2011 Funding (Year 2)

• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Assess and document current performance management and improvement efforts at the Utah Department of Health (UDOH) and local health departments
  • Build on Utah's strategic plan and map efforts
  • Support a process improvement activity to re-engineer the Utah Data and Reporting Tool, a peformance management tool used by both state and local health department programs
  • Implement workforce development activities for performance and quality improvement
  • Develop and deliver an annual Learning Congress
  • Provide intensive hands-on quality improvement (QI) training
  • Implement enterprise quality improvement project management software to build workforce capacity for QI efforts

2010 Funding (Year 1)

  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
