2012 Funding (Year 3)
- Amount of funding: $300,000
- Description of Project
Awardee will:- Continue collaboration with the Utah Association of Local Health Departments, which represents the state’s 12 local health districts
- Complete and begin implementation of a statewide health improvement plan
- Conduct accreditation readiness self-assessment and develop an accreditation roadmap
- Implement the following quality improvement projects: streamlining agency travel approval, improving quality culture at the Salt Lake Valley health department, and increasing reach and effectiveness of the smoking cessation program for pregnant women
- Provide management and performance improvement boot camp training
- Conduct a performance improvement project for online health assessments and lessons learned
2011 Funding (Year 2)
- Amount of funding: $300,000
- Description of Project
Awardee will:- Assess and document current performance management and improvement efforts at the Utah Department of Health (UDOH) and local health departments
- Build on Utah's strategic plan and map efforts
- Support a process improvement activity to re-engineer the Utah Data and Reporting Tool, a peformance management tool used by both state and local health department programs
- Implement workforce development activities for performance and quality improvement
- Develop and deliver an annual Learning Congress
- Provide intensive hands-on quality improvement (QI) training
- Implement enterprise quality improvement project management software to build workforce capacity for QI efforts
2010 Funding (Year 1)
Component I

- Description of Project:
Awardee will:
- Focus on building, institutionalizing, and implementing performance management capacity within health departments
- Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
