2012 Funding (Year 3)
- Amount of funding: $300,000
- Description of Project
Awardee will:- Coordinate performance management and quality improvement training and technical assistance across the department
- Establish a performance management tracking system
- Prepare for national public health accreditation by creating system-level changes to achieve national standards
- Establish a process to identify and address quality improvement opportunities within divisions and programs
- Identify promising and best practices and incorporate these practices into systems and methods to build capacity and contribute to the department’s evidence-based work
2011 Funding (Year 2)
- Amount of funding: $300,000
- Description of Project
Awardee will:- Utilize the expertise of the Performance Improvement and Accreditation Team (PIAT)
- Coordinate with PIAT and the Office of Performance Management to establish a quality improvement workgroup to coordinate performance management and quality improvement activities across the department
- Develop and institute a public health accreditation readiness system
- Prepare for accreditation by creating system-level changes to achieve national standards
- Establish a new process within programs to identify quality improvement gaps and prioritize activities
- Collaborate with federal, state, and local partners by establishing networks
- Identify promising and best practices and incorporate these practices into systems and methods as a crucial part of the department's capacity building process

- Description of Project:
Awardee will:
- Focus on building, institutionalizing, and implementing performance management capacity within health departments
- Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
