2013 Funding (Year 4)

  • Amount of funding: $500,000
  • Description of Project
    Awardee will:
    • Hire a school of public health to provide direct and remote technical assistance to local health departments (LHD) pursuing accreditation, focusing on review of quality of documentation to ensure the LHD can meet requirements for Public Health Accreditation Board standards
    • Complete an inventory of public health functions the state performs on behalf of locals (functions that locals cannot or choose not to perform)
    • Improve information technology governance and achieve efficiency and effectiveness by redesigning business processes around state health priorities, limiting the number of committees, and updating all policies and aligning these to state and federal standards 
    • Document the “as-is” manual processes for internal vital records data exchange with program operations and design a “to be” automated process for the same
    • Build return-on-investment capacity for chronic diseases

2012 Funding (Year 3)

  • Amount of funding: $500,000
  • Description of Project
    Awardee will:
    • Support local health departments wishing to meet the Public Health Accreditation Board standards
    • Develop a state health asset assessment and searchable database
    • Expand training in Lean Six Sigma to support quality improvement activities in laboratory operations and to improve the timeliness and accuracy of public health data
    • Build workforce capacity to conduct quality improvement and return on investment activities

2011 Funding (Year 2)

  • Amount of funding: $500,000
  • Description of Project
    Awardee will:
    • Advise the performance improvement team on the development and implementation of an internal self-assessment and the plan of action
    • Improve efficiencies in the Department of State Health Services contracting process
    • Create a single webpage for the Center for Health Statistics to improve access to health data
    • Improve data quality in the lead registry by leveraging process mapping results for births
    • Develop a dashboard for the birth defects, lead, trauma, and cancer registries using key performance measurement indicators
    • Improve state and local health department readiness for public health accreditation

2010 Funding (Year 1)

Component I

  • Amount of funding: $400,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
