2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Complete Public Health Accreditation Board (PHAB) coordinator training for two team members, including the accreditation coordinator, and initiate the collection of final examples of documentation for submission to PHAB
    • Conduct ongoing training on results-based accountability and values in action for its workforce and partners
    • Apply results-based accountability to improve health and agency business process and to improve performance and quality of internal and external services
    • Partner with internal and external stakeholders at the state and local levels to assess health and to develop, implement, and evaluate evidence-based and promising practices health improvement strategies for positive impact results and improved health equity
    • Maintain its Public Health Learning Collaborative, continuing to offer public health practice courses statewide—both online and in classroom—and increasing facilitator competencies

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Complete and submit the accreditation application to the Public Health Accreditation Board, including a state health assessment, the state health improvement plan, a strategic plan with a results improvement component, and a letter of support from the governor
    • Build quality and performance improvement competencies by providing two Results-Based Accountability (RBA) trainings for trainers and coaches and workshops for agency leaders and managers and key community partners
    • Identify opportunities for agency system and community health improvement using the RBA method
    • Identify a minimum of two program and system performance improvement opportunities, apply the RBA Performance Accountability process, and implement and track performance using appropriate data to evaluate and sustain improvement
    • Develop, publish, update, and promote public access to the New Mexico Results Scorecard, a web-based software program for internal and external data entry and viewing of population health improvement and program performance accountability reports
    • Further develop and maintain the Public Health Learning Collaborative to provide public health coursework for increased agency and community workforce competencies related to the 10 Essential Public Health Services and the knowledge and skills for effective public health practice

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Support development of a performance management system that will be used to conduct state and community-based partnership planning and to address accreditation standards and measures
    • Conduct a pre-application readiness assessment, gap analysis, and identification and collection of required documentation
    • Develop a plan for application submission, including a timeline and roadmap to achieving accreditation
    • Develop a sustainable department-wide performance improvement system that includes an education and practice program for its employees, using the Results Based Accountability (RBA) model, to improve health planning with other state agencies and community partners statewide and to examine and improve business practice that will ensure excellent customer services and effective management of the state's health resources
    • Develop a state health improvement plan by conducting a state and local planning process
    • Provide a train-the-trainer course to build performance improvement competencies for New Mexico Department of Health staff and community leaders
    • Use a performance management system to track and report the use of evidence-based policies, environmental approaches, and strategies to improve health outcomes
    • Provide training to increase public health competencies to evaluate the effectiveness of processes, programs, and interventions, and to identify areas for improvement
    • Collaborate with the University of New Mexico School of Medicine Public Health Program and New Mexico Public Health Training Center to develop courses to increase public health workforce competencies, including an evaluation workshop

2010 Funding (Year 1)

Component I

  • Amount of funding: $199,877
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
