2012 Funding (Year 3)

• Amount of funding: $657,600
• Description of Project
  Awardee will:
  • Complete and implement state health improvement and agency strategic plans
  • Complete the Public Health Accreditation Board self-assessment and action plan to identify and address gaps
  • Expand Health Policy Academy, Joint Public Health Data Center, and Syndromic Surveillance projects
  • Support local health departments in their preparation for potential public health accreditation
  • Implement quality improvement projects to improve 1) the Living Well chronic disease self-management program, 2) screening of target population for risk and fully engaging eligible families in Home Visiting Services screening target population, and 3) newborn screening confirmatory test response time

2011 Funding (Year 2)

• Amount of funding: $657,600
• Description of Project
  Awardee will:
  • Increase accreditation readiness of the state and local health departments
  • Enhance best practices implementation
  • Complete a comprehensive Mobilizing for Action through Planning and Partnerships (MAPP) process
  • Complete statewide health status, community themes and strengths, forces of change, and public health system assessments
  • Use the results of the process to guide the development of a statewide health improvement plan and a strategic plan
  • Provide grants and technical assistance to local health departments for them to update their MAPP processes, complete a community health improvement plan, conduct a more rigorous gap analysis of the Public Health Accreditation Board standards, or complete a quality improvement project
  • Expand the existing capability of their cardiovascular syndromic surveillance project by purchasing software that will enable them to capture and share real time syndromic surveillance data with hospitals and local health departments

2010 Funding (Year 1)

Component I

• Amount of funding: $200,000
• Description of Project:
  Awardee will:
  
  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

• Amount of funding: $1,000,000
• Description of Project:
  Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
