2013 Funding (Year 4)

  • Amount of funding: $937,858
  • Description of Project
    Awardee will:
    • Achieve national public health accreditation
    • Develop and implement its performance management system
    • Provide training, technical assistance, and funding to support local health jurisdictions’ completion of accreditation prerequisites and achievement of national standards
    • Develop and implement a framework for managing performance of Minnesota’s local public health system

2012 Funding (Year 3)

  • Amount of funding: $993,662
  • Description of Project
    Awardee will:
    • Apply for national public health accreditation
    • Implement a quality improvement (QI) plan
    • Provide QI training and technical assistance and accreditation prerequisite support to local health jurisdictions
    • Develop and implement a framework for managing performance of its public health system

2011 Funding (Year 2)

  • Amount of funding: $993,662
  • Description of Project
    Awardee will:
    • Develop a state health improvement plan
    • Work towards becoming accredited by forming a Minnesota Department of Health (MDH) accreditation team, implementing accreditation readiness quality improvement activities, and collecting MDH accreditation documentation
    • Assist local health jurisdictions and tribes in meeting the accreditation prerequisites and standards
    • Increase efficiencies and quality within the Environmental Health Division of MDH
    • Build a culture of quality at MDH
    • Support local cross-jurisdictional collaborative initiatives
    • Develop and implement information technology systems and standards to improve and expand cross-jurisdictional data sharing
    • Build a statewide performance management system
    • Implement an evidence-based approach to improve customer satisfaction at MDH

2010 Funding (Year 1)

Component I

  • Amount of funding: $300,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

  • Amount of funding: $1,660,128
  • Description of Project:
    Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
