2013 Funding (Year 4)

  • Amount of funding: $500,000
  • Description of Project
    Awardee will:
    • Implement a public health administration strategic plan and use results of a self-assessment against national standards to address public health administration performance gaps
    • Conduct a high-level systematic review of the its public health code to evaluate current effectiveness and identify opportunities for improvement
    • Conduct quality improvement activities to increase public health administration efficiency and effectiveness
    • Continue to offer quality improvement and performance management learning opportunities within its public health system
    • Provide performance improvement grants and technical assistance and consultation to local and tribal health departments

2012 Funding (Year 3)

  • Amount of funding: $500,000
  • Description of Project
    Awardee will:
    • Implement a state health improvement plan
    • Develop a Michigan Department of Community Health roadmap to accreditation
    • Conduct quality improvement initiatives to increase efficiency and effectiveness in the Sexually Transmitted Disease Program
    • Offer quality improvement and performance management learning opportunities within its public health system
    • Implement processes to improve customer satisfaction with public health programs and services

2011 Funding (Year 2)

  • Amount of funding: $500,000
  • Description of Project
    Awardee will:
    • Continue implementation of a statewide health assessment and health improvement planning initiative
    • Continue work on the department's strategic plan
    • Increase use of best practices
    • Implement quality improvement activities
    • Provide legal and policy training for staff
    • Refine reporting practices

2010 Funding (Year 1)

Component I

  • Amount of funding: $400,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
