2013 Funding (Year 4)

  • Amount of funding: $787,423
  • Description of Project
    Awardee will:
    • Finish Maine Center for Disease Control and Prevention (Maine CDC) strategic plan
    • Monitor implementation of the state health improvement plan and the Maine CDC strategic plan
    • Continue a partnership with Maine hospitals to streamline the state health assessment, state health improvement plan, and hospitals’ Internal Revenue Service requirements for community health needs assessments
    • Finalize documentation for accreditation
    • Streamline the local maternal and child health referral process
    • Increase breastfeeding
    • Improve access to public health data via Maine CDC’s website
    • Implement Maine CDC’s Idea System to encourage all staff to identify and act on opportunities for improvement
    • Increase the capability of Maine CDC staff and activities related to quality improvement (QI) through ongoing QI training
    • Complete implementation of a performance management system and identification of QI projects via this system

2012 Funding (Year 3)

  • Amount of funding: $843,182
  • Description of Project
    Awardee will:
    • Provide support and leadership for the development of the state health improvement plan
    • Support the development of the agency’s strategic plan
    • Complete mock assessment for public health accreditation to provide an outside review of readiness and gaps
    • Enhance information technology tools to create greater efficiencies in its Health Inspection Program
    • Streamline services provided via home visitation and public health nursing to improve alignment of services
    • Enhance its performance management system, performance scorecard, quality improvement training, and ongoing quality improvement projects

2011 Funding (Year 2)

  • Amount of funding: $843,182
  • Description of Project
    Awardee will:
    • Create a system to integrate, support, and routinely monitor quality improvement indicators and projects across the Maine Center for Disease Control and Prevention (Maine CDC), prioritze areas for improvement, and assure alignment with the agency's mission and priorities
    • Partner with Maine's Office of Information Technology to plan and implement  enhancements to Maine CDC's data systems, creating efficiencies through better coordination; for example, improved reporting mechanisms for health status data for the state and eight public health districts will be created via the Public Health Information Gateway
    • Support implementation of the organization's strategic priorities, completion of a full strategic plan, and creation of a state health improvement plan, as well as champion efforts to prepare for national accreditation

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

  • Amount of funding: $1,658,785
  • Description of Project:
    Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
