2012 Funding (Year 3)

• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Complete accreditation prerequisites to prepare for Public Health Accreditation Board application in 2014
  • Continue to collaborate with local health departments on quality improvement projects at the state and local levels
  • Provide performance management training to state and local health department staff
  • Collaborate with key partners, such as universities and training centers, to provide training and technical assistance to state and local public health staff for accreditation readiness, quality improvement initiatives, and strategic planning

2011 Funding (Year 2)

• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Provide basic and in-depth quality improvement (QI) training to state and local health department staff
  • Partner with local health departments regionally to enhance state health assessment/QI planning activities
  • Provide technical training and raise awareness of the Community Guide at the state and local level to increase implementation of best practices
  • Develop a user-friendly performance management dashboard, which displays information in a clear and concise fashion

2010 Funding (Year 1)

  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
