2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Facilitate the government-to-government relationships of the state health department and Kansas’ tribal sovereign nations to assess the interest of sovereign nations in the areas of community health assessment, strategic planning, development of an inter-tribal public health system, and readiness for public health accreditation
    • Expand the visibility of the statewide health dashboard, Kansas Health Matters, with targeted marketing strategies to increase the use of data, resources, and evidence-based health intervention strategies by multisector groups and the public
    • Develop and implement a quality improvement (QI) plan using the Turning Point performance management framework to identify gaps and opportunities for QI activities while conducting public health accreditation readiness activities
    • Strengthen Kansas public health workforce and training capacity by implementing a Train-the-Trainer Instructional Design course that focuses on development of relevant and effective training using evidence-based adult learning strategies
    • Conduct a survey of the public to determine the perception and use of public health services provided by the agency to guide future communication and outreach efforts

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Design a single web-based system portal (Kansas Health Matters) to provide data and information support to community health assessment (CHA)/community health improvement planning (CHIP) activities; this portal is anticipated to reduce local costs associated with CHA/CHIP processes
    • Develop an information system structure that includes state and local public health and external partners
    • Provide assistance and resources with other public and  private organizations to conduct community health needs assessments with community hospitals, local health departments, and other community health providers
    • Encourage hospitals and public health departments to conduct  collaborative CHA/CHIP activities to strengthen coordination of services and links between primary care and public health
    • Strengthen data collection and dissemination information systems used to identify health indicators
    • Train the public health workforce on use of evidence-based practices to address community health priorities identified in the planning process

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
