2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Monitor, evaluate, and revise the state’s health improvement plan and the department’s strategic plan
    • Provide quality improvement training and an open forum workshop on workforce issues for local public health agencies and other public health partners
    • Conduct performance improvement projects related to performance evaluations, code and administrative code review, and the department’s strategic planning, measurement, analysis, and knowledge management focus
    • Continue to strengthen procedures and methods of managing performance and assuring accountability at the local and state levels

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Develop, implement, and monitor action plans related to its strategic plan
    • Support training and assistance to local public health agencies on quality improvement and performance management
    • Conduct quality improvement initiatives related to behavioral health and emergency response
    • Strengthen performance accountability and performance management by improving capacity and methods for measuring performance and reporting results
    • Engage staff in efforts to address gaps in meeting public health standards

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Enhance statewide community involvement in state health improvement planning
    • Implement a sustainable strategic planning process and develop a strategic plan using business models and best practices
    • Assess capacity to meet public health standards and develop plans for making improvement
    • Engage in quality improvement activities at the local and state levels to address gaps in meeting public health standards

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
