2012 Funding (Year 3)

• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Develop, implement, and monitor action plans related to its strategic plan
  • Support training and assistance to local public health agencies on quality improvement and performance management
  • Conduct quality improvement initiatives related to behavioral health and emergency response
  • Strengthen performance accountability and performance management by improving capacity and methods for measuring performance and reporting results
  • Engage staff in efforts to address gaps in meeting public health standards

2011 Funding (Year 2)

• Amount of funding: $300,000
• Description of Project
  Awardee will:
  • Enhance statewide community involvement in state health improvement planning
  • Implement a sustainable strategic planning process and develop a strategic plan using business models and best practices
  • Assess capacity to meet public health standards and develop plans for making improvement
  • Engage in quality improvement activities at the local and state levels to address gaps in meeting public health standards

2010 Funding (Year 1)

  • Focus on building, institutionalizing, and implementing performance management capacity within health departments
  • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
