2013 Funding (Year 4)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Provide ongoing monitoring, technical assistance, and support for Chicago Department of Public Health (CDPH) programs through the performance and quality improvement (QI) system
    • Train all CDPH staff on QI basics through online QI 101 training series and at least 50 staff through QI learning collaboratives
    • Develop and implement a training plan for CDPH staff based on workforce public health competency assessment results and ensure all staff complete at least two public health competency trainings
    • Develop and implement management and leadership training series
    • Evaluate the performance and QI system, review and revise the QI plan, and implement outlined activities
    • Develop a plan to address all feedback from the Public Health Accreditation Board site review

2012 Funding (Year 3)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Achieve public health accreditation
    • Conduct quality improvement (QI) training and institute QI learning collaboratives to increase staff capacity to identify improvement opportunities and use QI tools
    • Evaluate the performance management system and implement improvements based on evaluation results
    • Use QI tools to improve administrative functions, such as contracting and hiring

2011 Funding (Year 2)

  • Amount of funding: $300,000
  • Description of Project
    Awardee will:
    • Develop two web-based reporting portals for contract agencies and for Chicago Department of Public Health (CPDH) staff to report monthly on performance measures and other interventions and/or implementation data
    • Increase performance management (PM) and quality improvement (QI) capacity
    • Improve efficiencies through technology enhancements and professional development
    • Increase CDPH's readiness for accreditation
    • Gain access to and provide training for staff on the City of Chicago's performance management data system
    • Conduct a staff knowledge assessment and devise a training plan to assist staff in better understanding that PM and QI are essential components for meeting and maintaining accreditation

2010 Funding (Year 1)

Component I

  • Amount of funding: $200,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC
